Release of Liability

I hereby make application to join a JCCC Cultural or Recreational class. Upon acceptance of my application, I agree to abide by the rules and regulations set by the JCCC in connection with the program.
I acknowledge that the activities taking place at the JCCC could cause personal injury including possible exposure to COVID-19 or other communicable diseases (collectively “Communicable Diseases”) which may result in serious illness and even death. I agree to follow all safety precautions, orders and guidelines promoted in an official capacity by the Chief Public Health Officer of Canada, Public Health Ontario including local Public Health Units while using JCCC facilities, including but not limited to requirements for physical distancing, face coverings, gathering limitations, hand washing and sanitization.
I hereby release the Japanese Canadian Cultural Centre, its directors, officers, employees, instructors, members, volunteers and invitees and licensees from any and all claims, demands, actions, causes of actions, or any other liability or obligation whatsoever arising out of or in connection with my participation in the any program of the JCCC, relating to exposure to or contracting Communicable Diseases, personal injury including, but not limited to, damage to or loss of property or otherwise, whether going to or away from or at or in the premises of the JCCC or elsewhere, and whether in contract or in tort.

Additional release for participants of minority age (Under age 18 at the time of registration)

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the rovisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases.

Furthermore, my child/ward understands and accepts these risks and responsibilities. I, for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
